Who might be able to join this trial:

• You are over 18 years old
• You have been diagnosed with severe or very severe COPD (a serious lung condition that makes breathing difficult) and/or are currently on or eligible for long-term oxygen therapy
• You have been diagnosed with Idiopathic Pulmonary Fibrosis (IPF), a condition that causes scarring of the lungs
• You have another type of lung scarring or inflammation disease (interstitial lung disease) that does not have a cure
• You have been diagnosed with a stage 3B or stage 4 solid tumor (an advanced cancer)
• You have serious heart failure that is either very severe (Class IV) or moderate-to-severe (Class III) with at least one hospital stay in the past year
• You do not currently have a signed advance directive (a legal document about your future medical wishes) on file with your doctor
• You are not on a waiting list for, and are not currently considering, a lung or heart transplant
• Your doctor believes there is a high chance you could become seriously ill within the next 2 years
• You are interested in thinking about completing an Advance Directive (a document that records your wishes for future medical care)

Who may not be able to join:

• Your condition may have treatments available that could extend your life (confirm with trial site)
• You are not able to speak and/or read English well enough to participate
• You are a brand new patient seeing your clinic doctor for the very first time
• You are currently being assessed for, or are already on a waiting list for, a transplant
• You already have a signed advance directive on file
• You have a cognitive (thinking or memory) impairment that means someone else would need to give consent on your behalf
